Waiver
By indicating your acceptance, you understand, agree, warrant and covenant as follows:

Waiver, Release, Indemnification and Hold Harmless Agreement I understand that golf activities have inherent risks and I hereby assume all risks and hazards incident to my participation in all Penn-York Junior Golf League activities. I further waive, release, absolve, indemnify and agree to hold harmless the Penn-York Junior Golf League, Bartlett Country Club, Elkdale Country Club, the Pennhills Country Club, Pine Acres Country Club, Wellsville Country Club, event sponsors, organizers, volunteers, supervisors, officers, directors, or participants, as well as, persons or parents transporting participants to and from activities from any claims or injury sus-tained during my participation in the Penn-York Junior Golf League activities.

The undersigned has read and voluntarily signs the release and waiver of liability and indemnity agreement, and further agrees that no oral representations, statements, or inducement apart from the foregoing written agreement have been made. The undersigned has also read the Standards of Play and Code of Conduct and Spectator Behavior (www.swnynwpa.com) and
agree to adhere to both at all times.
